Insurance Coverage for Tests

Planning to claim the cost of your diagnostic tests through private health insurance? We’re here to support you through the process and help make your insurance claim as smooth and straightforward as possible.

Getting Started

If you would like to claim your test through private health insurance, please contact your insurance provider before scheduling your test to confirm that your policy covers the required diagnostic service and to obtain pre-authorisation.

You can also email us at admin@gi-cognition.com if you need guidance with the process. Our team will help you with the information required by your insurer.

Covered Tests

Insurance coverage may apply to a range of diagnostic tests and specialist services we provide, including but not limited to:

• Breath tests (e.g. SIBO, fructose intolerance and lactose intolerance tests)
• Gastrointestinal function tests
• Other specialist diagnostic services

Coverage depends on your individual insurance policy, so please confirm eligibility with your insurer before proceeding.

We work with a number of leading UK private health insurance providers, including:

• Aviva
• WPA
• Vitality

What You’ll Need

Before your test is scheduled, please contact your insurance provider to:

1. Confirm that your policy covers the test you have been referred for.
2. Set up a claim, if required.
3. Obtain a pre-authorisation/authorisation number or code.

Once you have received your authorisation, please provide us with:

• Name of your insurance company
• Policy or membership number
• Claim reference number
• Pre-authorisation/authorisation code or number

Once we receive these details, we can communicate with your insurer regarding payment processing and any required documentation.

WPA Pre-Authorisation

If you are covered by WPA, please follow these steps:

1. Contact the WPA Claims Helpdesk on 01823 625270 to set up a claim or request pre-authorisation.
2. Request pre-authorisation for the test you have been referred for (for example, SIBO + IMO Lactulose Breath Test) and provide Dr Jafari’s Provider Number: 921597351.
3. WPA will provide you with a 6-letter pre-authorisation code.
4. Please email this code to admin@gi-cognition.com so we can proceed with your test.

Vitality Pre-Authorisation

If you are covered by Vitality, please contact Vitality to confirm your cover and request pre-authorisation for the test you have been referred for (for example, SIBO + IMO Lactulose Breath Test).

Once Vitality provides your authorisation details, please email the authorisation/reference number to admin@gi-cognition.com so we can proceed with your test.

Aviva Pre-Authorisation

If you are covered by Aviva, please contact Aviva to confirm your cover and request pre-authorisation for the test you have been referred for (for example, SIBO + IMO Lactulose Breath Test).

Once Aviva provides your authorisation details, please email the authorisation/reference number to admin@gi-cognition.com so we can proceed with your test.

Important Notes

• Insurance coverage and reimbursement depend on your individual policy terms and conditions.
• Some insurance providers may require a referral from your GP, specialist or consultant.
• Some policies may have excesses, limits or patient contributions.
• Please confirm your coverage and any applicable costs directly with your insurer before proceeding.
• Pricing may differ between insurance-covered services and self-pay options.
