If you are planning to use private health insurance to cover your test, please contact your insurance provider before scheduling your test to confirm that your policy covers the required test and to obtain a pre-authorisation/authorisation code.

Once we have received your referral, we will contact you to arrange your test. Please have the following information available:

• Your insurance company
• Your policy or membership number
• Your claim reference or pre-authorisation code

WPA Pre-Authorisation

If you are covered by WPA, please follow these steps:

1. Contact WPA through their Claims Helpdesk on 01823 625270 to set up a claim or request pre-authorisation.
2. Request pre-authorisation for the test you have been referred for (for example, SIBO + IMO Lactulose Breath Test) and provide Dr Jafari’s Provider Number: 921597351.
3. WPA will provide you with a 6-letter pre-authorisation code.
4. Please email this code to us so that we can proceed with your test.

Vitality Pre-Authorisation

If you are covered by Vitality, please follow these steps:

1. Contact Vitality through their member services or claims team to confirm your insurance cover and request pre-authorisation.
2. Request pre-authorisation for the test you have been referred for (for example, SIBO + IMO Lactulose Breath Test) and provide any provider details requested by Vitality.
3. Vitality will provide you with an authorisation/reference number if your request is approved.
4. Please email your authorisation details to us so that we can proceed with your test.

Aviva Pre-Authorisation

If you are covered by Aviva, please follow these steps:

1. Contact Aviva through their claims team to confirm your insurance cover and request pre-authorisation.
2. Request pre-authorisation for the test you have been referred for (for example, SIBO + IMO Lactulose Breath Test) and provide any provider details requested by Aviva.
3. Aviva will provide you with an authorisation/reference number if your request is approved.
4. Please email your authorisation details to us so that we can proceed with your test.

Important Information

Once we receive your pre-authorisation details, we can communicate directly with your insurance provider regarding payment processing where applicable.
